Standard Release for Alleluia! Lutheran Church's presentation of Power Lab from Group Publishing to be held July 21st-25th, 2008 at Dublin City Schools' Deer Run Elementary.

Medical Treatment

At check-in on Day 1, you will be asked to sign beside one of the following statements:

I GIVE consent for my child to receive emergency medical or dental treatment if I cannot be reached.
(If you sign this statement, you will also provide your child's preferred physician, dentist and hospital information as a condition of registration.)

-or-

I DO NOT GIVE consent for my child to receive emergency medical or dental treatment if I cannot be reached.

Photography

You will be asked to sign the following release for Alleluia to use photographs taken of VBS, which may include your child. If you do not sign the release, every attempt will be made to avoid taking or using pictures of your child, but some photographs may inadvertantly be taken.

I GIVE permission for photographs taken of my child at VBS to be used for purposes deemed appropriate by Alleluia! Lutheran Church. (Names will not accompany pictures.)

General Release

You will be asked to sign a general release of liability as a condition for your child's participation in VBS.

I hereby waive and release any and all rights and claims for damages I may have against Alleluia! and for any and all injuries or illness that may be suffered in connection with my child's involvement with Vacation Bible School.
